NOTICE OF SUMMONS TO WHOM IT MAY CONCERN, unknown biological father of the subject child, and anyone claiming to have parental interest in the minor child named above. The mother of said child is Charlie Theresa Ellis. YOU ARE FURTHER NOTIFIED that the above styled action seeking an Order terminating parental rights tot he above listed child was filed against you in the Athens-Clarke County Juvenile Court on April 24th, 2012, and an Order for Service by Publication was entered by the Athens-Clarke County Juvenile Court on June 7th, 2012. YOU ARE HEREBY COMMANDED AND REQUIRED to appear before the Juvenile Court of Clarke County, Georgia, located at 325 East Washington Street; Athens, Georgia, on the 2nd day of August at 9:00 o'clock, a.m. The hearing is for the purpose of determining whether or not your parental rights to the above listed child should be terminated. A copy of the Petition for Termination of Parental Rights may be obtained from the Clerk of the Juvenile Court at 325 E. Washington Street, Room 115, Athens, Georgia, 30601, during regular business hours, Monday through Friday, 8:00 a.m. until 5:00 p.m. exclusive of holidays. A free copy shall be available to the parents. Upon request, the copy will be mailed to the requester. The children are in the present physical custody of Athens-Clarke County Department of Family and Children Services. The general nature of the allegations is that all parental rights to the above listed child should be terminated. Said order to terminating parental rights is without limit as to duration and terminates all parents rights and obligations with respect to the child and all rights and obligations of the child to the parent arising from the parental relationship, including rights of inheritance. The parent is not thereafter entitled to notice of proceedings for the adoption of the child by another, nor has the parent any right to object to the adoption or otherwise to participate in the proceedings. YOU ARE FURTHER NOTIFIED that you lose all rights tot he child and will neither receive notice nor be entitled to object to the termination of your parental rights tot he above named child unless, within thirty (30) days of your receipt of this notice, you file: (1) A petition to legitmate the child pursuant to O.C.G.A. 15-11-96 et seq.; and (2) Provide notice of the filing of the petition to legitimate with the Court in which this action is pending. YOU ARE FURTHER NOTIFIED that while responsive pleadings are not mandatory they are permissible and you are encouraged to file with the Clerk of this court and serve upon Petitioner's attorney, Randall H. Forester, P.O. Box 1985, Athens, Georgia, 30603, an answer or other responsive pleading within sixty (60) days of the date of the order for service by publication.
