The mother of the child is Jennifer Hendrick.<br /><br /> YOU ARE FURTHER NOTIFIED that the above-styled action seeking custody of the above-named child was filed against you in the Juvenile Court of Elbert County on or about the 5th day of June, 2012, and that by reason of an order for service by publication entered by the Court on or about the 23rd day of July, 2012;<br /><br /> YOU ARE HEREBY COMMANDED AND REQUIRED to appear before the Juvenile Court of Elbert County, Georgia, located at the Franklin County Justice Center, 7085 Highway 145, Carnesville, Georgia on the 4th day of September, 2012, at 9:30 o lock a.m., for the purposes of a final hearing to determine whether or not the above-named child is deprived and, if so, what disposition should be made. <br /><br /> YOU ARE FURTHER NOTIFIED that a provisional hearing will be held before the Juvenile Court of Elbert County, Georgia, located at the Hart County Courthouse, Courthouse Square, Hartwell, Georgia 30643, on the 11th day of June, 2012, at 9:30 a.m. for purposes of an adjudicatory and dispositional hearing and located at the Hart County Courthouse, Courthouse Square, Hartwell, Georgia, on the 27th day of June, 2012, at 9:30 o lock a.m. Findings of fact and orders of disposition made pursuant to the provisional hearings will become final at the final hearing unless you appear at the final hearing.<br /><br /> You are further notified that you are required to provide to the Court, within ten days from the date of preliminary disposition in this case, the names and addresses of all relatives and other persons who might be considered as possible placements for the child.<br /><br /> A copy of the petition may be obtained from the Clerk of the Juvenile Court at the Elbert County Courthouse, 12 South Oliver Street, Elberton, Georgia, during regular business hours, exclusive of holidays. A free copy shall be available to the parents. Upon request, the copy will be mailed to the requester. The child is in the present legal and physical custody of the Elbert County Department of Family and Children Services.<br /><br /> The general nature of the allegations are: abandonment, neglect and deprivation.<br /><br /> YOU ARE FURTHER NOTIFIED that while responsive pleadings are not mandatory, they are permissible, and you are encouraged to file with the clerk of this court and serve upon Petitioner attorney, Nadine D.
